State-wide Equipment Program

Last year SWEP launched a revised Home Modification Services for both the Aids and Equipment Programs and the National Disability Insurance Scheme, which includes a newly appointed Builders and Tradesperson Panel.
This was released to assist Prescribers, Clients, Participants and Proprietors in utilising the new system effectively.
The process for our clients stays the same, you need to see your doctor or treating specialist and get a referral to go see an Occupational Therapist who is registered with SWEP.
They will then talk to you about the new model and applicants have three funding options to choose from
  • Option A - Supply Only of Assistive Equipment.
SWEP can assist with funding towards assistive equipment and you arrange for its installation by a suitably qualified Builder or Tradesperson
  • Option B - Simple Non-Structural Home Modifications under $1,300
SWEP can assist with funding towards assistive equipment and its installation.
  • Option C - Complex Home Modifications over $1,300
SWEP can assist with funding towards assistive equipment and its installation for complex non-structural or structural modifications to your home.

Once you have found the model that meets your needs your Occupational Therapist will work with you and submit an application.
