Applying for Continence Items

How do I apply?

You need to be assessed by an appropriate health professional/Assistive Technology (AT) Practitioner (continence nurse, etc.) from your local area who will help you determine the most suitable continence item needed. This includes an assessment of whether you or your carer can safely use the continence item. Your health professional/AT Practitioner will complete the online form for the item that is needed.


What happens when the assessment is done?

Once the assessment is completed, your healthcare professional/AT Practitioner will submit the application and supporting documents to SWEP electronically on your behalf.

What happens when the application has been processed?

Once your eligibility is confirmed, SWEP will order the required continence items from the supplier which will be delivered to your nominated address. Please note, a wait list may apply if funds are not available. A waitlist does not apply to existing consumers on the program. SWEP will advise you if your application will be placed on a waiting list.
