The State-wide Equipment Program (SWEP) provides subsidised funding towards the cost of assistive technology equipment and related products. We support people in Victoria with an eligible disability or ongoing health or ageing related need to live independently.
About SWEP
SWEP is a business unit of Grampians Health Ballarat and is funded by the Department of Health, Victoria.
Our goal is to support eligible Victorians to improve independence in their home, enhance community participation and support families and carers in their role. This is achieved through the provision of a broad range of subsidised assistive technology; home & vehicle modifications; oxygen equipment; continence, lymphoedema compression garments & laryngectomy items/products. Referral to our program is via a suitability qualified and skilled Assistive Technology (AT) Practitioner (Occupational Therapist, Physiotherapist, Podiatrist, Pedorthist, Respiratory Physician, Continence Nurse, Lymphoedema Practitioner, Speech Pathologist), who is registered to prescribe to our service. Your Practitioner will assess your equipment/product needs and apply for funding on your behalf.
The SWEP team is based in Ballarat, supporting consumers across the State of Victoria. Our admin team receives thorough, ongoing training to ensure they can provide the highest standard of service.
