SWEP will be introducing the below listed components of the new home modification model in the coming months in two distinct stages:
Stage 1
What needs to occur in preparation for the new home modification model?
To assist with the implementation of the new home modification model, SWEP will be writing to all Clients that are on the A&EP waitlist to introduce the new model and to determine if the prescription we have on file is relevant to their current needs. The needs of Client's that are frail aged and/or have disabilities have the capacity to change at any time, resulting in the submitted prescription no longer being appropriate. Changes in needs resulting from illness, deterioration in physical and cognitive functioning, relocation to alternative accommodation such as a residential facility, all have the capacity to make the submitted prescription no longer appropriate.
As a Prescriber how can you help?
As a Prescriber you have the capacity to assist with the transition to the new model. If you have submitted a home modification prescription and you know the Client no longer requires the service, simply inform SWEP of this by email on SWEPHighcomplexity.org.au
Your assistance in identifying Clients that no longer require the service will assist SWEP in prioritising and delivering the new model more effectively to Clients that are currently in need.
