How to check your eligibility for support from the Victorian Aids & Equipment Program

The questions below will help you determine your eligibility for support. The green boxes mean you continue onto the next question, the red boxes may advise you to seek further clarification to see if you are eligible to apply for support from the Aids and Equipment Program.


1. To be eligible for support from our programs you must:

  • be a permanent resident in Victoria or
  • hold an Australian Government Visa or
  • be an Asylum seeker or
  • hold a temporary protection Visa

    And

     Have a disability of a permanent nature or are frail aged and living at home

  No
   to either -
please call SWEP PH: 1300 747 937 (select option 6) 
   to discuss your eligibility with our Customer Service Team. If you
              are not eligible for support through our programs, you may be 
              able to access support through other
Assistive Technology Programs

           


2. If you have been treated as a public hospital in-patient, have you been discharged for more than 30 days? 

 Yes  
  
go to question 3


  No
 
you may still be eligible to apply after the 30 day discharge period. Please call
  SWEP PH: 1300 747 937 (select option 6) to discuss your eligibility with our 
             Customer Service Team.


3. Are you pursuing a claim in respect of your disability with any of the following:

  • Victorian WorkCover Authority (VWA)
  • Transport Accident Commission (TAC)
  • Legal Compensation Claim
 
No
to all of the above - go to question 4


 Yes
  to any of the above - please call SWEP PH: 1300 747 937 (select option 6) to 
  discuss your
eligibility with our Customer Service Team.


4. Are you under the age of 65?

  
No
go to question 5



  Yes
   go to question 4a

4a. Are you a Participant receiving support from the NDIS? 

   
No
go to question 5


 Yes
   If you are an NDIS participant please contact our Safety Link Assist                   (SLA)Team on PH: 1800 995 009 to discuss how we can support you.


5. Do you hold a Department of Veterans Affairs card (DVA)?      

 No 
 go to question 6


 Yes
 Gold Card holders are only eligible to apply for a power wheelchair or a scooter.
                       



6. Are you receiving an Australian Government Home Care Level Package?     
 No  
 you may be eligible to apply for support through our programs. 
 *If you have private health insurance see below


Yes
  If you are recipient of a Level 1 or 2 package, you may still be eligible for
  support through our programs. If you are a recipient of a
Level 3 or 4 package,
             you may still be able to apply for h
ome modifications, a power wheelchair/
             scooter
.

*Please note: you have private health insurance your fund may assist to cover the costs of the AT item either in part or in full.  If they do not cover the total cost or they decline to cover the AT item requested you may be eligible to apply for support through our programs
