Applying for Laryngectomy Consumables

How do I apply?

You need to be assessed by a SWEP registered Speech Pathologist from your local area who will help you determine the most suitable products/items needed. 

Once the assessment is completed, your Practitioner will complete the online application form and supporting documents for the products that you need on your behalf.   Your Practitioner will also need to source a quote for the products/items from a supplier.

What happens when the application has been processed?

Once your eligibility is confirmed, SWEP will order the products from the product supplier, who will deliver them to your nominated address.

If the subsidy does not cover the full cost of the products, a gap funding form will be sent to you to complete. This form will advise SWEP how you wish the remainder of the cost of the products to be paid.
