You need to be assessed by a SWEP registered practitioner (Australasian Lymphology Association (ALA) accredited practitioner, including OT, Physiotherapist, Registered Nurse, Medical/Nurse practitioner, Massage Therapist OR Osteopath) from your local area who will help you determine the most suitable compression garments needed.
Once the assessment is completed, your Practitioner will complete the online application form and supporting documents for the garments that you need, on your behalf. Your Practitioner will also need to source a quote for the garments from a supplier.
Once your eligibility is confirmed, SWEP will order the compression garments from the product supplier, who will deliver them to your nominated address.
If the subsidy does not cover the full cost of the garments, a gap funding form will be sent to you to complete. This form will advise SWEP how you wish the remainder of the cost of the garments to be paid.
