Dear Practitioners
The SWEP Lymphoedema program administration team have noticed an increase in incomplete applications being submitted and this causes unnecessary delays for our consumers and extra administration work for staff, suppliers and practitioners.
The most common reasons for an application being deemed incomplete are listed below. Please review this list as it is important in helping us to reduce delays in processing your requests.
- Eligibility form - Recent issues include:
- all sections of the Confirmation of Disability (CoD) form are not completed
- when the consumer has selected no to being Victorian Resident instead of yes
- CoD is not signed by consumer or PoA/ NoK (verbal consent not permitted)
- absence of Home Care Package information. If consumer is on a HCP, we need to know what level to check eligibility. This is also a question in the pre-screening section of the application
- Quote - all sections on the SWEP quote template are mandatory but commonly the below are incomplete:
- affected limb and whether its left, right or bilateral
- delivery fee not included. If a delivery fee is not applicable, please note this on the quote
- total GST component not specified on the quote and in the application (not just ' inclusive 10% " ™)
- Quote does not include what Australian or International Standard it meets (i.e. TGA, RAL, Oeko-Tex)
- a new quote is provided for each application. Please contact the supplier to obtain a new quote as we are unable to process an application to order if the same quote number has previously been used
- Application:
- incorrect picklist code used which does not match the quoted item
- whether the garment is for left, right or bilateral and what limb is affected. Please ensure this is included on the quote as well as in the online application under ' consumer diagnosis " ™
- costs - when filling out the pricing on the quote section of your application please ensure the amounts are put in the correct sections and avoid combining garment costs with the delivery costs and the split of the GST
