SWEP has completed a review of how we
manage home modifications applications and has recognised the current process
using an approval in principle (AIP) model requires changing. We are therefore
streamlining processes and documentation, to allow us to appropriately assess
the request on receipt of application.
We have worked with our Clinical Advisors
and staff to review feedback received from practitioners and consumers which
has resulted in the removal of the AIP model.
We acknowledge that the AIP process has added unnecessary layers of
complexity for practitioners and SWEP in submitting and managing home
modifications applications.
Effective immediately, all home
modification applications submitted via an application on the portal, will
require a new form called the 'Homeowner Consent for Home Modifications' found on the SWEP website here
The new consent form will confirm the
consumer, supplier/builder and practitioner details, address of property and
information on the modification required.
Documents required when submitting your application are;
>Home modifications valued up to $2,000
Due to the impacts of the COVID-19 Pandemic during 2020 and 2021 we have had long delays in progressing home modification applications, due to access for home visits, builder availability etc. We are actively working on clearing these requests upon receipt of relevant documentation and have funding allocated to progress these applications to order.
If you are aware of any incomplete home modification applications, please up-date all necessary documentation (or contact us if you are unsure) so that these applications can be progressed as quickly as possible.
This new process will be applied to any
current AIP applications. If you have any queries or feedback please email swep@bhs.org.au
